If you are new to our practice, we need you to complete paperwork before you are seen. You may arrive early and do this at our facility or download forms below and complete them before you arrive.

If you would like to talk with Dr. Glen before he examines your smile and provides you with information specific to your teeth, you may request a (gratuitous) new patient consultation. This get-to-know-you step is not necessary, but available for potential new patients.
